Review Evidence and Findings

What it does

Each review cycle carries an evidence library and a findings list. Evidence records what the committee looked at; findings record what it concluded — strengths, risks, opportunities, requirements, or observations — each with a priority and an optional link back to its evidence.

Who it's for

Cycle managers and edit-rights members add evidence and findings; other members read them subject to privacy level; organization administrators also get org-wide libraries searching across every cycle.

How it works

Evidence. The Evidence card on the cycle page lists everything attached to the review. Each row shows a title, an optional summary, and a privacy badge; form evidence with a linked assignment also gets a View full submission button. Adding evidence offers four types — Manual summary, Form, Metric, and Document. Choosing Form adds a Form assignment ID field, so the item points at a real assignment instead of a retyped copy. New and edited evidence always saves at Committee privacy; the panel has no control for a different level.

Privacy levels.

  • Public and aggregated — visible to everyone who can open the cycle.

  • Committee — visible only to cycle members and cycle managers. This is what any evidence added through the panel gets.

  • Restricted and admin-only — visible only to people who can manage the cycle.

Findings. The Findings card sits next to evidence, sorted critical-first. Each finding has a type (strength, risk, opportunity, requirement, or observation), a priority from low to critical, a title, a description, and a status from the same eight-value vocabulary used by goals and action items. You can pick one evidence item from the Linked evidence dropdown so the conclusion traces back to its data — no more links can be added once saved. A Create goal from finding button starts a new goal, pre-filled with the finding's title and text.

Evidence items and findings both move through work statuses. The cycle overview flags evidence that has not yet been marked complete, and the "needs your attention" panel counts it toward the review's next step.

Before you start

  • Adding or editing evidence and findings needs cycle management access — scoped review-management permission, or membership with Can edit on.

  • For form evidence, have the form assignment's ID ready before you start the Add evidence form; the field rejects a blank value.

Do this

  1. Open the review cycle's page (/reviews/:cycleId) → you should see the Evidence and Findings cards side by side.

  2. In the Evidence card, select the Add summary button → the panel switches to the Add evidence form.

  3. Fill in the Title field and pick a type from the Evidence type dropdown → for Form, a Form assignment ID field appears.

  4. Add a Summary, then select the Save evidence button → you should return to the Evidence card and see the new item with a Committee privacy badge.

  5. In the Findings card, select the Add finding button → the panel switches to the Add finding form.

  6. Fill in the Title, pick a Type and Priority, and, in the Linked evidence dropdown, select the evidence item you just added → confirm it shows instead of No evidence link.

  7. Select the Save finding button → you should return to the Findings card and see the new finding with its type and priority badges.

You're done when

  • ✓ The evidence item shows in the Evidence card with the type and privacy you expect.

  • ✓ The finding shows in the Findings card with its priority badge, and, if you linked one, its evidence.

  • ✓ Selecting Create goal from finding on that finding opens a pre-filled goal on the improvement plan.

Boundaries and limits

  • Adding or editing evidence and findings requires cycle management access (scoped review permission or membership with edit rights). Ordinary members read only.

  • The evidence forms only offer Manual summary, Form, Metric, or Document, and always save at Committee privacy — no in-form control for other levels.

  • A form evidence item needs a real, existing form assignment ID; the platform requires it for the Form type but does not validate its format.

  • Privacy levels are enforced at the data layer, not just hidden in the interface: a committee-level item is simply absent for non-members.

  • Findings have an Archive action that removes them from the active list while keeping the record. Evidence items have no equivalent delete or archive action.

  • Required evidence from the review type is tracked as report blockers, separate from this library — attaching evidence does not auto-resolve a blocker; a manager resolves or waives it explicitly.

  • The org-wide evidence and findings libraries are paginated and searchable but scoped to your organization only.

Common questions

Q: Can we attach the actual survey results instead of retyping them? A: Yes. Add evidence of type Form and paste the form assignment's ID; the review points at the live record instead of a copy.

Q: Who can see committee-only evidence? A: Only members of that review cycle and people who can manage it. Administrators who can merely view the cycle's scope do not see committee-level items unless they are members or managers.

Q: Does a finding need evidence? A: No. The Linked evidence dropdown is optional, but linking it lets anyone reading the finding trace it to the data behind it.

Q: We marked all evidence complete — why does the report still show blockers? A: Blockers are resolved separately on the readiness tab. Completing evidence review does not auto-resolve the required-evidence blockers created at cycle setup.

Q: I want to change an evidence item's privacy level. Where do I do that? A: The evidence form doesn't currently expose that control — new and edited items save as Committee. Talk to your administrator if a different level is needed.
