Open Forms and Evaluation Requests

What it does

Open forms let people start a form themselves instead of waiting to be assigned one, such as logging an on-the-spot evaluation after a procedure. Evaluation requests work the other way: you ask a specific person to complete a form about you, and they accept or decline. Both are self-service paths that create normal, trackable assignments.

Who it's for

Learners and faculty starting evaluations and self-assessments on demand or requesting feedback about themselves, with program administrators controlling which forms allow these paths.

How it works

Starting an open form. The Open Forms page lists every published form you're eligible to start on demand. Starting one prompts for whatever the form needs: a recipient (when it's about a person), nothing extra for a self-assessment (automatically about you), and sometimes the related rotation, course, session, event, or shift. Once started, the form behaves like any other assignment and appears in your My Forms list.

Requesting an evaluation. On the request page, choose a form that allows requests, search for the person you want to complete it, and optionally add a short message. The responder can accept (turning it into an in-progress form) or decline, optionally with a reason. People already asked for the same form are flagged during selection so you don't double-request.

Administrators enable these behaviors per form: an on-demand toggle makes the form startable by responders, and a separate "allow recipients to request" option enables the request flow.

Before you start

  • The form you want must already be published with on-demand access turned on — check the Open Forms page (/forms/open) to confirm it's listed.

  • Know the activity the form concerns (rotation, course, session, event, or shift) if the form asks for one.

  • For a request instead, know who you want to complete it — you'll search for that person by name.

Do this

  1. Open the Open Forms page (/forms/open) → you should see Available Forms with every form you're eligible to start.

  2. Select the Start button on the form you want → a setup card may ask for a recipient or a related activity, depending on how the form was built.

  3. Answer that setup card → the form opens for completion on the completion screen and behaves like any other assignment.

  4. To request a form instead, open the Request Form page (/forms/request) and select New Request → the Request Form dialog opens with a Form field.

  5. Choose the Form field, search for and select the Responder field, and optionally add a message on the same dialog → select Send Request → the request shows as sent, and the responder can accept or decline it.

You're done when

  • A started open form appears on your My Forms page as a normal assignment.

  • A sent request shows as pending until the responder accepts (moves to in progress) or declines.

Boundaries and limits

  • Only published forms with an active on-demand setting appear on the Open Forms page; eligibility is scoped to your program and institution.

  • The request flow is only available for forms where the administrator has enabled recipient requests.

  • Requests can be time-boxed: a form may only accept requests within a configured window around your scheduled rotation (for example, within 7 days of the rotation's end). Outside the window the request is rejected with an explanation.

  • If a form enforces one submission per schedule, a second request or submission for the same person and the same rotation is blocked.

  • A person-directed open form requires choosing a valid recipient from the offered list; you cannot free-type an arbitrary recipient.

  • Declined requests are final for that request; you would need to send a new one.

Common questions

Q: Why doesn't a form I expect appear on my Open Forms page? A: Either the form is not published, on-demand access is not enabled for it, or it is not available to your program. Your program administrator controls all three.

Q: Can the person I send a request to say no? A: Yes. Responders can decline a request, optionally with a reason, and the request shows as declined.

Q: Why was my request rejected with a message about a time window? A: That form only accepts requests near the start or end of your rotation. Try again inside the stated window.

Q: Do open forms show up for administrators? A: Yes. Starting an open form or accepting a request creates a standard assignment that appears in the program's assignment list.
