What it does
Program administrators can import already-completed evaluation responses for a published Rotation, Course, or Program form template from a CSV file. The import creates completed, locked assignments in one pass, without sending submission alerts or spawning reciprocal evaluations.
Who it's for
Program administrators and forms coordinators who need to backfill historical or externally collected evaluations into Oracle — for example after switching over from a previous system, or when a partner site returns completed spreadsheets.
How it works
On Form Templates, open a published Rotation, Course, or Program template and choose Import CSV. The wizard has three steps — Upload, Preview, and Result. Each row is keyed, so re-importing the same file for the same batch skips rows that already landed instead of duplicating them.
Before you start
The template must be published and be a Rotation, Course, or Program type — no other types are supported.
You need permission to manage form assignments for the template's program or institution.
Prepare your file within the limits: 500 data rows, 5 MB, and answers under 10,000 characters.
Do this
Open Forms → Templates (
/forms/templates) and open a published Rotation, Course, or Program template.Select Import CSV → the import wizard opens on the Upload step.
Select Download sample CSV to get the template's columns, fill it in, then select Choose file and pick your filled file → the wizard checks the rows and moves to the Preview step.
Review the rows on the Preview screen → rows Oracle could not resolve by email and context show a reason; ready rows show as Ready.
Select the Import rows button, which shows the ready row count → the Result step shows how many evaluations landed and how many were skipped, then select Done.
You're done when
The Result step reports a count of imported evaluations that matches your ready rows.
The imported evaluations show as completed, locked assignments on the assignments list.
Re-running the same file reports the already-imported rows as skipped, not duplicated.
Boundaries and limits
Only Rotation, Course, and Program form types are supported in this release.
Sync import is capped at 500 data rows and 5 MB per file. Wider forms may use up to 200 columns; long-text answers up to 10,000 characters.
Imports write completed, locked evaluations. They do not create in-progress drafts.
Import does not send submission alert emails and does not create reciprocal counterpart assignments.
You need permission to manage form assignments for the template’s program or institution.
Skipped rows are reported with reason codes only — answer text is never written into error reports.
Common questions
Q: Can I import Event or Self forms? A: Not in this release. Use Rotation, Course, or Program templates, or assign those other types through the normal assignment flows.
Q: What if some people or rotations are missing? A: Those rows are skipped with a clear reason in the preview. Fix the spreadsheet (or create the missing people/schedules) and import again — already-imported rows will not duplicate.
Q: Does import notify evaluators or evaluatees? A: No. Bulk import is for completed historical data and does not dispatch submission alerts.
